Diabetes Education Program

​

Our Diabetes Education Program consists of a Registered Nurse and a Registered Dietitian in each clinic who work together to provide education and counselling to patients who are living with diabetes, who have pre-diabetes, or who have been newly diagnosed with Type 2 Diabetes.

​

Our Diabetes Education Program can help you:

  • Identify personal goals

  • Manage your diabetes – including your sugar levels, foot and eye care, lifestyle changes, blood pressure, and healthy eating habits.

Pharmacist

​

Our clinic pharmacist dispenses information, not medication.  She does not fill prescriptions. Our pharmacist can answer questions like:

​

  • What is the best time to take my medication?

  • Will my medicines interact with each other?

  • It is hard for me to pay for my medication, what are my alternatives?

  • What about non-prescription drugs or natural medicines?

  • Can I use my drug insurance?

  • I want to quit smoking, where can I get help?

 

To make an appointment with our pharmacist, please call our reception desk. No referral is needed.

Shared Mental Health Care Team

​

The Shared Mental Health Care team is made up of psychiatrists, two social workers, an advanced practice nurse, a Registered Nurse, and an administrative coordinator. The team works with your family doctor and other members of the Family Health Team to help patients ages 16 years and older with mental health problems.

​

Here is what our Shared Mental Health Care Team can do for you:

​

  • Individual and group counselling on psychological issues

  • Early detection of mental health issues

  • Coping with mental illness

  • Mental health medication management

  • Accessing mental health resources in the community

  • The team also offers group sessions on depression and anxiety

  • Diagnostic Clarification

 

At this time, the Shared Mental Health Care Team only sees Ontario patients who are 16 years or older who have been referred to them by a family doctor or resident.

Social Workers

​

We provide psycho-social assessments and support to patients of both the Riverside and Civic Family Health Teams. There is a part-time Social Worker at the Riverside campus and a full time Social Worker at the Civic campus. We provide services to youth over 12 y/o, individuals, couples, and families. We see people reporting symptoms of mild to moderate anxiety and/or depression. We help people access resources for financial, housing, transportation, and employment issues. We help people with future planning such as accessing retirement living, long term care or naming Powers of Attorney for Personal Care and Finances. We see people dealing with grief and loss issues or chronic health issues as this too is a loss. We assist people experiencing physical, sexual, emotional, and/or financial abuse by another person. We provide the following modes of therapy: Cognitive Behavioural Therapy (CBT), Emotion Focused Therapy (EFT), Brief, Narrative and Relational Therapy, Dialectical Behavioural Therapy (DBT), Solution Focused Therapy and Trauma Informed therapy. Assessments, referrals and supports are also provided for people with eating disorders of any kind.

Program Activities/Offerings and Descriptions:

 

Once an appointment is made with our Social Workers they will assess your counselling, resource or support needs, follow you for regular appointments, make appropriate referrals to other required services identified, determine with you how many sessions will be required, collaboratively set realistic and measurable goals for therapy and/or support, and facilitate an agreed upon and planned ending to the social work intervention with you.

 

How can the program be accessed:

 

Speak to your doctor if you are in emotional, financial, family or work distress so they can make a referral to the Social Worker. If you have been followed by the current Social Worker at the Family Health Team, simply contact them by phone or email to ask for a resumption of service.

 

Registered Dietitians

​

Our dietitian provides individual and family nutrition counselling to help patients make informed choices about the food they eat and translate nutritional information into realistic advice.  Here’s what our Registered Dietitians can do for you:

​

  • healthy eating on a budget

  • food allergies and intolerances

  • nutrition for a healthy pregnancy

  • healthy vegetarian eating

  • nutrition for infants and children

  • vitamins and minerals for good health

  • Nutritional Management of Chronic Diseases

 

Please note that you do not need a referral to see a Registered Dietitian. If you would like to book an appointment with one of our Registered Dietitians, please contact the front desk.

Partnerships

​

                                                                            Community Care Access Centre (CCAC)

​

The Ottawa Hospital Academic Family Health Team currently has a partnership with the Community Care Access Centre (CCAC) of Ottawa. The CCAC helps people of all ages with disabilities, the elderly, those who are too ill to remain in their own homes, and those who require special care after a hospital stay before they are able to return home. The CCAC also ensures quality health care to school-age children who have special medical needs.
